Remote Nurse Health Specialist

Overview

Alignment Health is seeking a remote nurse health specialist to provide triaging service for patients, focusing on seniors and the chronically ill, in a collaborative virtual care environment.

In Short

• Provide triage services for patients in a virtual care center.
• Collaborate with physicians and nurse practitioners for patient care.
• Manage patient care and treatment through telehealth services.
• Conduct follow-up calls and virtual visits.
• Review patient vitals and support disease management.
• Educate patients on disease processes and medication compliance.
• Work with Electronic Medical Records.
• Available for various shifts including overnight and weekends.
• Requires California LVN/RN License.
• Possibility of engagement extension based on performance.

Requirements

• Minimum 2 years of experience as LVN with ICU, ER, or Med-Surg experience.
• High School Diploma or GED; completion of an accredited nursing program.
• CPR certification required.
• Effective communication and problem-solving skills.
• Knowledge of clinical standards of care.
• Bilingual skills (Spanish/English) preferred.
• Willingness to obtain licenses in additional states preferred.
• Experience in gerontology or hospice care preferred.
• Ability to work in a fast-paced environment.
• Mathematical skills for calculations and statistics.
