Overview

The Primary Care Physician at Oak Street Health is responsible for providing equitable and effective value-based healthcare to local Medicare patient populations at neighborhood primary care centers, focusing on delivering high-quality care.

In Short

• Provide primary care in a neighborhood clinic.
• Assess and diagnose patients.
• Oversee and administer primary care services.
• Prescribe medications and treatments.
• Maintain electronic medical records.
• Collaborate with care teams to meet health quality goals.
• Work with Practice Managers to manage care teams.
• Encourage new graduates to apply.
• Possibility of a Center Medical Director position for qualified candidates.
• Full-time schedule, Monday through Friday.

Requirements

• Medical Doctor (M.D.) or Doctor of Osteopathy (D.O.) degree.
• Board Certification in Internal Medicine or Family Medicine.
• Active, unrestricted State License and DEA license.
• US work authorization.
• Bilingual proficiency preferred.
• Experience in outpatient primary care settings.
• Experience with geriatric populations is preferred.
• Fellowship training in Geriatrics is a plus.
• Experience in a Value-Based Healthcare Model.
• Collaborative working experience for positive health outcomes.
