Overview

The Analyst, Case Management role at CVS Health involves utilizing critical thinking and judgment to facilitate appropriate healthcare outcomes for members through care coordination, support, and education.

In Short

  • Engages members in their healthcare decision-making.
  • Documents care in compliance with guidelines.
  • Requires travel up to 50-75% of the time.
  • Must reside in North/West side of Chicago, IL.
  • Full-time position with a 40-hour work week.

Requirements

  • Bachelor's degree in a human services field required.
  • 2+ years of experience in behavioral health or social services.
  • Case management and discharge planning experience preferred.
  • Managed care experience is a plus.
  • Valid IL Driver's License required.
  • Reliable transportation necessary.
  • Ability to travel as needed.
  • Strong communication and motivational skills.
