Overview

This is a telework position with a travel requirement to meet members face to face. As a Case Manager Analyst, you will facilitate appropriate healthcare outcomes for members by providing care coordination, support, and education through the use of care management tools and resources.

Requirements

  • 2+ years of experience in behavioral health, social services, or human services.
  • 2+ years of case management experience.
  • Must reside in Macon, Moultrie, Piatt, or Champaign Counties, IL.
  • Willing and able to travel 50-75% of the time.
  • Discharge planning experience preferred.
  • Managed care experience preferred.
  • Microsoft Office experience preferred.
  • Bachelor's degree or non-licensed master level clinician required.
