Director of Clinical Care - Remote

Overview

This is a remote role that requires infrequent travel to New York. The ConcertoCare Director of Clinical Care develops and implements organization-wide and market-specific medical expense initiatives as a collaborative effort in partnership with Medical Directors, Population Health leaders, and our local market frontline care teams. Our DCCs support rising risk and high-risk patients through engagement and care delivery interventions to prevent unnecessary ER visits and hospitalizations in real-time and lead our market high-risk huddles to proactively address drivers of utilization and improve clinical outcomes. S/he partners with our finance and analytics team members to develop and interpret clinical analytics and real-time data dashboards and deep dive analyses that allow for proactive strategies to manage avoidable medical expenditures and reactive deep dive investigations to understand opportunities for clinical outcomes optimization.

• Oversee and manage clinical utilization outcomes and KPIs.
• Employ data and effective clinical tools to identify areas of utilization risk.
• Identify and target high-risk patients in need of clinical attention.
• Be a liaison between acute, subacute, and skilled care institutions.
• Facilitate multi-disciplinary high-risk huddles.
• Utilize tools to identify areas of high-cost utilization.
• Collaborate with other providers to evaluate patient needs.
• Provide direct patient care to avoid emergency room admissions.
• Support training of providers and staff in care processes.
• Participate in organizational DCC collaboration and Medical Expense Review processes.

Requirements

• Nurse Practitioner or Physician Assistant with national board certification.
• Active clinical licensure in applicable state(s).
• Active DEA licensure and state-specific controlled substance registration.
• Certification in basic life support (BLS) required.
• Clinical experience caring for aging adults and complex care needs.
• Previous experience with managed care and/or value-based healthcare.
• Demonstrated success in matrixed organizations.
• Strong communication, facilitation, and data analysis skills.
• Average to advanced data analytics and computer skills required.
• Ability to travel as needed; proof of insurance required.
