Overview

The LTSS Service Care Manager for Behavioral Health is responsible for developing and coordinating holistic care management activities for members with significant mental and behavioral health needs, ensuring quality and cost-effective healthcare outcomes.

• Remote position with up to 50% travel required.
• Coordinates care management activities for members with mental/behavioral health needs.
• Develops and assesses care plans and educates members and families.
• Acts as a liaison between members, families, and healthcare providers.
• Performs site visits to assess member needs.
• Provides education on healthcare topics and community resources.
• Maintains documentation in the electronic care management system.
• Ensures compliance with clinical guidelines and state/federal regulations.
• Requires a Master's degree in Mental Health or Social Work.
• Licensed Behavioral Health Professional or RN required.

Requirements

• Master's degree in Mental Health or Social Work.
• 2-4 years of related experience.
• Licensed Behavioral Health Professional or RN with BH experience.
• Ability to travel up to 50%.
• Experience in crisis intervention and care management.
