Overview

The Care Manager (RN) is responsible for developing and facilitating complex care management activities aimed at providing high quality and cost-effective healthcare outcomes for members with physical needs.

In Short

Requirements

  • Degree from an Accredited School of Nursing or Bachelor's in Nursing.
  • 2-4 years of related experience.
  • RN - Registered Nurse State Licensure required.
  • Ability to travel up to 75% within designated regions.
  • Strong communication and coordination skills.
  • Knowledge of social determinants of health.
  • Experience in care management is preferred.
  • Ability to work independently and as part of a team.
  • Proficiency in documenting care management activities.
