Overview

The Care Manager is responsible for developing, assessing, and facilitating care management activities focused on mental and behavioral health needs, ensuring high-quality healthcare outcomes for members.

In Short

  • Develop and facilitate care management activities for mental and behavioral health members.
  • Conduct assessments via phone or home visits.
  • Create personalized care plans for members.
  • Coordinate care between members, families, and healthcare providers.
  • Monitor and update care plans based on member needs.
  • Educate members and families about treatment options and healthcare benefits.
  • Document and maintain compliance with regulations.
  • Travel up to 75% for home visits in Hudson and Union County.
  • Requires a Master's degree in Behavioral Health or Social Work.
  • Must have relevant licensure as a Behavioral Health Professional or RN.

Requirements

  • Master's degree in Behavioral Health or Social Work.
  • 2-4 years of related experience.
  • Licensed Behavioral Health Professional or RN.
  • Experience with mental health and substance use disorders.
  • Strong communication and interpersonal skills.
  • Ability to travel within designated regions.
  • Knowledge of healthcare compliance standards.
  • Proficient in care coordination and patient education.
  • Ability to work independently and as part of a team.
