In Short

• Full-time position with day shift hours, no evenings, weekends or holidays.
• Approximately 20% remote work and 80% direct patient care.
• Conduct comprehensive assessments and develop individualized care plans.
• Collaborate with internal teams and external partners for high-quality care.
• Assist in managing transitions to and from inpatient settings.
• Proactively engage with providers to identify high-risk patients.
• Utilize management tools for effective transitional care management.
• Communicate effectively with stakeholders to promote core values.
• Travel throughout the market area as needed.
• Salary range of $65,000 - $80,000 based on experience.

Requirements

• Registered Nurse or Licensed Social Worker.
• 3-5 years of experience in case/care management or care coordination.
• Experience with the Dementia population is required.
• Formal training in Dementia is highly sought.
• Valid driver’s license required.
