In Short

• Assesses psychosocial status of patients related to illness and environment.
• Plans interventions based on social evaluations.
• Maintains clinical records on all patients referred to social work.
• Provides information and referral services for patients and families.
• Serves as liaison between patients and community agencies.
• Maintains collaborative relationships with healthcare personnel.
• Participates in the development of total care plans.
• Assists in understanding social and emotional factors related to health.
• Participates in discharge planning.
• Participates in inservice education programs.

Requirements

• Must have a Master’s degree in Social Work (MSW).
• Experience in healthcare or home health settings preferred.
• Strong communication and interpersonal skills.
• Ability to work collaboratively with a healthcare team.
• Knowledge of community resources and services.
