Person-Centered Planning (PCP) Specialist - Remote

Overview

The Person-Centered Plan (PCP) Specialist is responsible for supporting the coordination of services for up to 75-85 people, facilitating the development of service plans that reflect individual’s unique strengths, needs, preferences, and aspirations.

In Short

• Facilitates development of personalized PCPs.
• Ensures timely completion of all PCPs.
• Collaborates with regional teams to review service usage.
• Maintains routine PCP-related documentation.
• Tracks the PCP process in the agency’s information system.
• Monitors LTSS daily for pending plan activity.
• Engages with service providers to resolve issues.
• Stays current on DDA required PCP processes.
• Demonstrates understanding of agency’s mission and values.
• Performs other related duties as needed.

Requirements

• Degree in human services, social work or related field.
• Minimum 3 years of experience with adults with disabilities.
• Creative, flexible, and detail-oriented.
• Proficient analytical and problem-solving abilities.
• Strong organizational and time management skills.
• Outstanding communication and interpersonal skills.
• Comfortable with technology, including Microsoft Office Suite.
