Overview

As a Clinical Care Specialist at 9amHealth, you'll provide personalized, virtual care and education to patients managing chronic cardiometabolic conditions, working closely with our physicians and care team.

In Short

• Deliver expert care, education, and lifestyle support for patients managing chronic conditions.
• Develop consult notes that align with internal protocols.
• Partner with Patient Care Advocates to coordinate care.
• Design personalized nutrition and lifestyle plans.
• Collaborate with the pharmacist team on medication-related questions.
• Monitor patient progress through follow-ups.
• Apply motivational interviewing techniques.
• Help patients interpret health data and lab results.
• Maintain accurate documentation of care provided.

Requirements

• Registered Dietitian Nutritionist with active licensure.
• Certified Diabetes Care and Education Specialist (CDCES) or eligibility to obtain certification.
• At least 5 years of clinical experience in managing chronic conditions.
• Proficiency in medication management and EMR systems.
• Experience in telehealth or virtual care delivery.
• A quiet, professional home workspace.
• Strong communication skills and a patient-centered approach.
