Remote Cardiometabolic Registered Nurse (Full Time)

Overview

9amHealth is specialized cardiometabolic care, focusing on a whole body approach to preventing and treating obesity, diabetes, high blood pressure, and high cholesterol. We are seeking a Registered Nurse to deliver patient-centered health coaching and chronic disease management.

In Short

• Deliver patient-centric health coaching integrating medical needs with preferences.
• Triage patient-reported symptoms and escalate to Providers when appropriate.
• Provide education on diabetes, hypertension, hyperlipidemia, and weight management.
• Coordinate care between patients, providers, and clinical teams.
• Create consults aligning with internal protocols for Providers.
• Document patient care accurately to meet internal standards.

Requirements

• Strong empathy and communication skills.
• Bachelor of Science in Nursing (BSN) required.
• Active Registered Nurse license in good standing.
• Certified Diabetes Care and Education Specialist (CDCES) or willingness to pursue.
• 3+ years of direct patient care in an ambulatory or telehealth setting.
• Adaptable and comfortable working independently.
• Proficiency with digital tools for care delivery.
• Flexibility for rotational weekend or holiday coverage.
• A HIPAA compliant workspace.
