Overview

You Health Medical Groups is seeking a dedicated and compassionate Registered Nurse (RN) to join our dynamic team as a part-time Care Coach, facilitating communication between patients and healthcare providers through the Hims & Hers telehealth platform.

In Short

• Provide individualized health coaching tailored to patients' needs.
• Communicate empathetically to address patient concerns.
• Utilize protocols to ensure compliance in care delivery.
• Serve as a primary contact for telehealth services.
• Document patient care in EMR systems.
• Conduct ongoing patient care duties and special projects.

Requirements

• Active Registered Nurse (RN) license.
• Diploma in nursing; Bachelor’s preferred.
• Strong interpersonal and communication skills.
• At least two years of clinical experience.
• One year of telehealth experience preferred.
• Proficiency in telecommunication technology and EMR systems.
• Excellent organizational skills.
• Knowledge of healthcare regulations (HIPAA).
• Direct patient care experience with Mental Health and/or Weight Management preferred.
