Overview

The Registered Nurse at Oak Street Health is responsible for coordinating patient care and providing a seamless experience while ensuring competent nursing care through clinical assessment and education.

In Short

• Build strong relationships with patients and their support teams.
• Provide competent nursing care and support preventative care.
• Respond to telephonic requests and manage symptom triage.

• Conduct patient education and care both in-person and remotely.

Requirements

• Active Registered Nurse (RN) Licensure.
• BLS Certification.
• Experience with Electronic Medical Records (EMR).
• Strong communication and assessment skills.
• Ability to maintain patient confidentiality.
• US work authorization.
• 2+ years of healthcare experience preferred.
• Fluency in additional languages is a plus.
