Overview

The purpose of a Primary Care Physician at Oak Street Health is to provide equitable and effective value-based healthcare to local Medicare patient populations at our innovative network of neighborhood primary care centers.

In Short

• Provide primary care to Medicare patients.
• Operate at a single medical center with large care teams.
• Focus on delivering quality care rather than volume of services.
• Assess and diagnose patients at local clinics.
• Prescribe and administer treatments and medications.
• Maintain Patient Electronic Medical Records.
• Collaborate with leadership to meet health quality goals.
• Work with Practice Managers to manage the care team.
• New graduates are encouraged to apply.
• Highly qualified candidates may be considered for a Center Medical Director position.

Requirements

• Medical Doctor (M.D.) or Doctor of Osteopathy (D.O.)
• Graduate of Internal Medicine or Family Medicine
• Board Certification or board eligible
• Active, non-probationary, unrestricted State License
• Active DEA license
• US work authorization
• Fellowship training in Geriatrics preferred.
• Experience with Geriatric populations preferred.
• Experience in a Value-Based Healthcare Model preferred.
• Bilingual proficiency where applicable.
