Overview

This position is a field-based Case Manager Registered Nurse role at CVS Health, focusing on providing care coordination and support to waiver members in South Cook County and surrounding areas.

In Short

• Field-based position with 50-75% travel required.
• Manage a full caseload of waiver members.
• Conduct comprehensive evaluations of members' needs.

Requirements

• 2+ years of clinical experience.
• Active Illinois RN license in good standing.
• Reliable transportation for travel.
• Experience in Case Management and Discharge Planning preferred.
• Managed Care experience is a plus.
• Proficient in Microsoft Office.
• Associates degree minimum; BSN preferred.
