Overview

We are seeking a self-motivated, energetic, detail-oriented, highly organized, tech-savvy Case Management Coordinator to join our Case Management team at CVS Health, where we focus on delivering enhanced human-centric health care.

In Short

• Coordinate care for members through telephonic and face-to-face assessments.
• Manage a caseload of supportive and medically complex members.
• Develop proactive action plans to enhance member wellness.
• Determine appropriate services based on health needs.
• Work in a flexible Monday to Friday schedule.
• Must be bilingual in English and Spanish.
• Travel 50-75% to meet members in Miami Dade county.
• 1+ year experience in behavioral health or long-term care.
• Computer proficiency in Microsoft Office.
