LPN Coordinator Full Time - Remote

Overview

VitalCheck Wellness is seeking a Licensed Practical Nurse (LPN) to join our team within our corporate office to provide coordination for our specialty programs.

In Short

• Coordinate between patient, clinical and operations partners to set up appointments.
• Ensure high adherence rate to the program.
• Provide education around diabetes management and exercise advice.
• Support clients in diabetes management and well-being.
• Establish and maintain rapport with patients.
• Maintain clients documentation.

Requirements

• Bachelor’s degree in nursing or related field.
• Active LPN license in NY or NJ preferred.
• 3-5 years of experience in nursing or health administration.
• Experience with educating on chronic conditions.
• Proficient with Microsoft Office and EMR systems.
• Strong communication skills.
• Bilingual in Haitian Creole or Spanish preferred.
• Ability to work independently.
