The Facilitator, Telemedicine Impact Plus (TIP) plays a crucial role in coordinating care for patients with complex conditions, ensuring they receive appropriate services and support in collaboration with healthcare professionals.

Requirements

• Bachelor's degree in Nursing or Allied Health Profession.
• 4-6 years of practical and related experience.
• Valid Ontario Driver's License.
• Knowledge of healthcare legislation and collaborative patient service delivery.
• Ability to think strategically for patient care services.
• Expertise in caring for vulnerable clients and chronic conditions.
• Skills in adult education techniques.
• Strong information technology and computer skills.
• Case Management Certificate is an asset.
• Experience with telemedicine and geriatrics/community health is a plus.
