Facilitator, Telemedicine Impact Plus (TIP) - Remote

Overview

The Facilitator for Telemedicine Impact Plus (TIP) at UHN plays a crucial role in ensuring that patients with complex co-morbid conditions receive coordinated care and services in partnership with primary care practitioners, with the ultimate goal of preventing avoidable emergency department visits and hospitalizations.

• Utilizes clinical knowledge and educator skills to support patient care.
• Works collaboratively with healthcare teams and patients.
• Proactively sources and pre-screens referrals for consultations.
• Initiates patient contact promptly after referral.
• Performs comprehensive clinical assessments of patients.
• Coordinates the development of care plans with primary care practitioners.
• Participates in training and education processes.
• Supports program development and evaluation.
• Ensures all documentation is completed as required.
• Works in a remote capacity with home visits.

Requirements

• Bachelor's degree in Nursing or a related field.
• 4 to 6 years of practical and related experience.
• Valid Ontario Driver's License and access to a vehicle.
• Solid knowledge of healthcare legislation and practices.
• Ability to think strategically in patient care.
• Experience with complex co-morbidities and chronic mental health challenges.
• Knowledge of adult education techniques.
• Strong IT and computer skills.
• Case Management Certificate is an asset.
• Experience with telemedicine is an asset.
