In Short

• Provide virtual individual cognitive assessment & care plan services.
• Interpret cognitive assessment tests remotely for Medicare-eligible individuals.
• Develop and document personalized care plans based on assessment results.
• Work in compliance with Medicare billing requirements.
• Collaborate with behavioral health professionals.
• Maintain accurate documentation in an electronic health records system.
• Must have an active, unrestricted medical license (MD or DO).
• At least 2 years experience in relevant fields preferred.
• Comfort with telehealth platforms and EHR.
• Flexible schedule and remote work options available.

Requirements

• MD or DO with an active, unrestricted medical license.
• At least 2 years experience in neurology, geriatrics, psychiatry, primary care, or a related field preferred.
• Familiarity with cognitive testing tools such as MoCA, MMSE, or equivalent.
• Strong communication skills and a patient-centered approach.
• Must have experience working with clients who are age 65+.
• Must be able to pass a criminal background check.
• Must have an active NPI.
• Existing credentialing with Medicare is a plus.
• Must be comfortable utilizing technology and providing teletherapy.
• Available to start with a minimum of 5 hours per week.
