Overview

As a Dementia Care Physician Assistant at Synapticure, you will diagnose, treat, and manage dementia and other acute episodic illnesses through comprehensive care visits, utilizing telehealth and collaborating with an interdisciplinary care team.

• Diagnose and manage dementia and acute illnesses.
• Conduct telehealth visits with patients and caregivers.
• Facilitate annual visits to monitor dementia care plans.
• Provide education and management skills to caregivers.
• Order and interpret lab tests and adjust treatment plans.
• Collaborate with neurologists and primary care physicians.
• Maintain compliant documentation in EMR.
• Support strategic initiatives regarding the GUIDE program.
• Ensure high customer satisfaction and retention.
• Practice ethically and follow regulations.

Requirements

• Masters of Science in Physician Assistant Studies.
• Current Physician Assistant credentialing body certificate.
• Active, unrestricted Physician Assistant licensure.
• 2 years of experience in primary care or neurology.
• Knowledge of geriatrics and dementia care.
• Excellent communication and organizational skills.
• Ability to work in a fast-paced, team-based environment.
• Experience with electronic medical records.
• Ability to assess psychosocial needs in a diverse environment.
• Comfort with multiple communication platforms.
