Overview

The Care Consultant plays an integral role in assessing the needs of individuals and their living environment, determining service eligibility, care planning, and coordinating person-centered services for older adults.

Requirements

• Knowledge of Medicare and Medicaid.
• Experience in client assessment and case management.

• Ability to meet deadlines and handle multiple tasks.
• Willingness to submit to background checks.
• Ability to adjust schedule to meet client needs.
• Experience in training new team members.
