Remote Licensed Clinical Social Worker (LCSW)

Overview

Salvo health is looking for an experienced LCSW/LICSW to play an integral role on our care team, providing personalized psychosocial support and counseling to patients.

In Short

• Conduct comprehensive psychosocial assessments of patients.
• Provide exceptional counseling and health education.
• Develop personalized care plans with patients.
• Support goal setting for chronic condition management.
• Document patient interactions in the EHR system.
• Coordinate with the clinical team for patient advocacy.
• Offer crisis intervention and support.
• Create consumer-facing content on mental health best practices.
• Translate complex medical ideas for a lay audience.
• Work with a cross-functional product team to improve patient experience.

Requirements

• LCSW license with practice in NY, FL, and MI.
• 2+ years of experience as an LCSW.
• Bachelor’s and Master’s degrees required.
• Experience with telehealth platforms preferred.
• Experience with GI conditions or weight management a plus.
• Excellent interpersonal communication skills.
• Strong organizational skills.
• Ability to thrive in a fast-paced environment.
• Remote work experience preferred.
