Part-Time Registered Nurse Care Manager - Remote

Overview

Privia Health is seeking a Part-Time Registered Nurse Care Manager to provide on-demand telephone-based care management services through a 24/7 Nurse Advice Line, aiming to improve patient care and reduce unnecessary visits to clinics and emergency departments.

In Short

• Handle inbound communications from patients seeking information about symptoms or care concerns.
• Conduct outbound communications for follow-up and care coordination.
• Consult and coordinate with team members to assess and plan patient care.
• Assist with finding appropriate providers and coordinating appointments.
• Record member data in medical record systems.
• Provide health information and coaching to members.
• Comply with HIPAA regulations.
• Work remotely with flexible scheduling primarily during nights and weekends.
• Maintain a minimum of 16 hours of coverage per week.
• Privia will assist in setting up the home office environment.

Requirements

• Current RN licensure in assigned state(s).
• Bachelor’s degree required.
• 3-5 years of recent clinical experience.
• Experience in call centers or triage positions preferred.
• Exceptional customer service skills.
• Strong computer and internet skills.
• Clear communication and listening skills.
• Self-motivated and disciplined.
• Ability to work in a quiet home office environment.
• Bilingual candidates preferred (Spanish, Korean, Vietnamese, or Farsi).
