Centralized Follow-Up Advanced Practice Clinician (APC) - Remote

Overview

The Centralized Follow-Up (CFU) Advanced Practice Clinician (APC) at Planned Parenthood of Greater New York will provide virtual care focused on reproductive and sexual health, including abortion and gender-affirming care, while utilizing the EPIC Electronic Health Record system.

In Short

• Provide patient care in a virtual environment.
• Utilize EPIC for managing patient records and interactions.
• Handle prescription requests and manage abnormal lab results.
• Consult with physicians to develop treatment plans.
• Participate in onboarding and training of new staff.
• Work collaboratively with CFU team members and other departments.
• Demonstrate commitment to health equity and patient-centered care.
• Manage urgent labs and expedite patient scheduling.
• Provide excellent customer service to internal and external customers.
• Work flexible hours, including weekends.

Requirements

• Strong knowledge of sexual and reproductive health.
• Experience with abortion care and LGBTQ populations.
• Excellent communication and interpersonal skills.
• Proficiency in Microsoft Office and familiarity with Microsoft Teams.
• Bilingual in Spanish preferred.
• Ability to work under pressure and manage time effectively.
