In Short

• Full-time Lead Care Manager position.
• Engage with individuals living with Serious Mental Illness/Substance Use Disorder.
• Utilize evidence-based approaches to promote health goals.
• Conduct telephonic and SMS outreach for follow-ups.
• Collaborate with multidisciplinary teams.
• Maintain a caseload of individuals.
• Assist individuals in securing community support.
• Work primarily from home with occasional on-site visits.
• Bilingual in English/Spanish preferred.
• Salary range of $22-$25/hour.

Requirements

• 1+ years of general work experience (Case Management preferred).
• Located in California.
• Bilingual – English/Spanish.
• Strong understanding of cultural fluency.
• High degree of empathy.
• Ability to work collaboratively in a multidisciplinary team.
• Organizational skills.
• Strong technical skills and comfort with technology.
• A valid driver’s license and auto liability insurance.
