Overview

As a Registered Nurse – Clinical Case Consultant in the CalAIM Enhanced Care Management program, you will be a catalyst for better health outcomes among California’s vulnerable populations.

In Short

  • Provide clinical oversight to care coordinators and case managers.
  • Conduct case consultations to ensure care plans are safe and evidence-based.
  • Support seamless transitions of care from hospital to home.
  • Collaborate with various health providers and organizations.
  • Maintain accurate documentation of clinical reviews and member interactions.
  • Lead trainings for non-clinical staff on chronic disease management.
  • Analyze outcomes to identify trends and opportunities for improvement.
  • Contribute to policy development for clinical best practices.
  • Mentor care team members to enhance their clinical understanding.
  • Promote accountability and data integrity across the team.

Requirements

  • Registered Nurse license in California.
  • Experience in clinical case management.
  • Knowledge of chronic disease management and mental health.
  • Strong communication and collaboration skills.
  • Ability to analyze clinical outcomes and improve processes.
  • Experience with electronic health records (EHR).
  • Training or mentoring experience preferred.
  • Commitment to culturally responsive care.
  • Understanding of Medi-Cal ECM standards.
  • Flexibility to adapt to changing healthcare environments.
