Weekend Remote Care Coordinator

Overview

Open Arms Solutions is a premier private duty home care provider dedicated to helping seniors live independently and with dignity in the comfort of their own homes in the Chicago area. We are looking for a compassionate and driven Community Care Coordinator to join our team and help us deliver exceptional care and support to our clients and caregivers on Saturdays and Sundays in a remote capacity.

In Short

• Serve as the primary weekend liaison with community partners.
• Collaborate with senior care providers for coordinated care.
• Conduct remote consultations and assessments with clients.
• Guide clients through the intake and registration process.
• Manage a weekend caseload as the main point of contact.
• Coordinate emergency coverage for caregivers.
• Support weekend hiring and staffing decisions.
• Match caregivers to clients based on various factors.
• Conduct virtual quality assurance check-ins with clients.
• Act as a dedicated liaison between clients and teams.

Requirements

• 3–5 years of experience in a client-facing role.
• Proven experience in care management and client relationships.
• Strong communication and problem-solving skills.
• Knowledge of ADLs and IADLs for senior care clients.
• Experience in a private duty/home care service company.
• Exceptional organizational skills and attention to detail.
• Proactive, service-oriented mindset.
• Proficiency in care management software and Microsoft Office 365.
