Nurse Clinician - Heart Failure, Outpatient - Remote

Overview

The Nurse Clinician will work in a multidisciplinary environment, utilizing nursing skills to perform patient assessments and implement nursing interventions. This role requires exceptional interpersonal skills and the ability to establish effective relationships with patients and healthcare providers.

In Short

• Hybrid Schedule; Monday to Friday 8AM to 5PM; No Holidays, No Weekends, No On-call; 2 days/week remote post-orientation
• Perform patient assessments via phone or in person
• Provide pre-attending care and document nursing assessments
• Perform routine nursing duties including phlebotomy and medication administration
• Interpret test results and take appropriate actions
• Communicate and collaborate with other departments
• Triage and prioritize incoming telephone calls
• Manage urgent medical situations according to protocols
• Initiate and coordinate patient education
• Maintain compliance with policies and procedures related to patient care

Requirements

• Associate’s degree in Nursing
• Graduation from an accredited nursing school
• State Licensed Registered Nurse
• Certification in Basic CPR
• 2 years of relevant clinical experience
• Bachelor’s degree in nursing preferred
