Registered Nurse - IRIS Self-Directed Personal Care (SDPC) - Remote

Overview

This position is for a Registered Nurse licensed in Wisconsin to support individuals enrolled in the IRIS program, providing personal care oversight and guidance.

In Short

• Remote position with home-based work.
• Partner with individuals in the Wisconsin IRIS program.
• Provide personal care assessments and oversight.
• Develop individual plans of care.

• Provide education and training for participants and care providers.
• Regularly scheduled visits with clients in their homes.
• Must have a current Wisconsin RN license.
• Minimum 2 years of nursing experience required.

Requirements

• Associates Degree in Nursing required.
• Experience with home health and individuals with disabilities preferred.
• Proficiency in Microsoft Office and data entry skills.
• Excellent written and verbal communication skills.
• Strong teaching and mentoring abilities.
• Good organizational and time management skills.
• Current unrestricted RN license in Wisconsin.
• Valid Driver’s License required.
• Bachelor’s Degree in Nursing preferred.
• Experience with Wisconsin Medicaid Personal Care Program preferred.
