Registered Nurse - IRIS Self-Directed Personal Care (SDPC) - Remote

Overview

This position is for a Registered Nurse licensed in Wisconsin to support individuals enrolled in the IRIS program, providing personal care oversight and guidance.

In Short

• Remote position with scheduled home visits.
• Provide oversight and guidance to IRIS program participants.
• Administer the Wisconsin Personal Care Screening Tool.

Requirements

• Associates Degree in Nursing required.
• Minimum 2 years of nursing experience, with home health experience preferred.
• Proficiency in Microsoft Office and data entry.
• Excellent written and verbal communication skills.
• Strong teaching and mentoring abilities.
• Good organizational and time management skills.
• Current unrestricted RN license in Wisconsin.
• Valid Driver’s License required.
• Bachelor’s Degree in Nursing preferred.
• Experience with Wisconsin Medicaid Personal Care Program preferred.
