IRIS Consultant - Remote

Overview

The IRIS Consultant position involves partnering with individuals enrolled in the Wisconsin IRIS Program to help them navigate long-term care options and achieve their personal goals.

In Short

• Remote position with home-based office.
• Regular visits with IRIS participants in their homes and communities.
• Build relationships and assist participants in navigating the IRIS program.
• Help develop customized plans for achieving goals related to various life aspects.
• Document all activities and maintain compliance with program requirements.
• Research community resources to support participants' outcomes.
• Educate participants on budget management and program compliance.
• Collaborate with other staff to implement participants' plans successfully.
• Attend monthly team meetings and weekly check-ins.
• Must have a valid driver's license and reliable transportation.

Requirements

• Bachelor’s degree in social work, psychology, or related field, or equivalent experience.
• 1+ year of experience in social services for individuals with disabilities and older adults.
• Ability to work independently and be self-motivated.
• Knowledge of Long-Term Care programs and self-determination principles.
• Excellent problem-solving and critical thinking skills.
• Strong time management and prioritization skills.
• Ability to maintain professional relationships and work with diverse populations.
• Resourceful with knowledge of community resources.
• Proficient in various technologies including Outlook, Teams, and Excel.
