Overview

The IRIS Consultant position involves building relationships with individuals enrolled in the Wisconsin IRIS Program, helping them navigate long-term care options, and supporting their personal goals.

• Remote position with home-based office.
• Regular visits with IRIS participants in their homes and communities.
• Assist participants in navigating the Wisconsin IRIS program.

• Document all activities and maintain compliance with program requirements.

• Attend monthly team meetings and weekly check-ins.
• Must have a valid driver's license and reliable transportation.

Requirements

• Bachelor’s degree in social work, psychology, or related field.
• 1+ year of experience in social services.
• Knowledge of Long-Term Care programs.
• Excellent problem-solving and time management skills.
• Ability to work independently and maintain professional relationships.
• Resourceful with knowledge of community resources.
• Proficient in technology including Outlook, Teams, and Excel.
• Comfortable working with diverse populations.
