IRIS Consultant - Remote

Overview

The IRIS Consultant position involves partnering with individuals enrolled in the Wisconsin IRIS Program to help them navigate long-term care options and achieve their personal goals.

In Short

• Remote position with home-based office.
• Regular visits with IRIS participants in their homes and communities.
• Assist participants in identifying and achieving long-term care goals.
• Build supportive relationships with diverse individuals.
• Document planning activities and participant progress.
• Educate participants on budget management and resources.
• Collaborate with other staff for successful plan implementation.
• Attend monthly team meetings and weekly check-ins.
• Requires a Bachelor’s degree or equivalent experience.
• 1+ year of experience in social services preferred.
