Overview

The Nurse Case Manager plays a crucial role in supporting patients and healthcare providers by facilitating access to therapies and providing ongoing support throughout the patient journey.

In Short

  • Provide information on Patient Support services and therapies.
  • Assist patients and caregivers with assessments related to services.
  • Act as a liaison between patients and healthcare providers.
  • Document patient interactions within electronic records.
  • Collaborate with reimbursement specialists to secure funding.
  • Participate in specialized training and attend relevant meetings.
  • Coordinate treatment logistics with healthcare providers.
  • Train new Nurse Case Managers and provide feedback.
  • Report adverse events as per program procedures.
  • May be required to be on standby/on call.

Requirements

  • Registered Nurse (RN) or Registered Practical Nurse (RPN) diploma.
  • Five years of nursing experience with a focus on healthcare and case management.
  • Bilingualism in French and English strongly preferred.
  • Experience in oncology is an asset.
  • Strong patient focus and critical thinking skills.
  • Effective communication skills, both oral and written.
  • Proficiency in Microsoft Office programs.
  • Ability to work under pressure and multi-task.
  • Strong attention to detail and organizational skills.
  • Leadership and interpersonal skills.
