Included Health is looking for a compassionate and dedicated Physician generalist who is fellowship trained in Palliative Care to join our Care & Case Management (CCM) Program, where you will play a vital role in supporting members throughout their healthcare journey.

• Provide holistic, patient-centered care through remote consultations.

Requirements

• Active medical license and Board Certification in Family Medicine or Internal Medicine.
• Additional certification or experience in Palliative Care preferred.
• 3-year Family Medicine or Internal Medicine residency or equivalent.
• 2-years of post-residency experience providing direct care to patients.
• Willingness to maintain licensure in multiple states.
• IMLC state residency is important.
• Preference for candidates with multiple state licenses.
