In Short

• The Collaborating Physician works with an APN performing in-home and telehealth wellness assessments.
• Available via telecommunication or in-person for consultation with APN.
• Regularly performs annual chart reviews.
• Consults with APN to ensure adherence to policies and quality.
• Accountable for outstanding customer service.

Requirements

• Active/unrestricted Medical Physician License.
• Board certified/board eligible required.
• Residency completion required.
• 1 year experience in Healthcare management.
• Experience with Risk Adjustment.
• Must reside in the state where the openings exist.
