Our work began with caregiving support for employees and their loved ones—now we’re extending that care to individuals on Medicare and the people who support them. We’re blending clinical expertise with whole-person care to help people manage health and social needs with less stress and greater confidence.

This is care, reimagined. And we’re looking for dedicated Nurse Practitioners or Physicians Assistants to help us bring it to life.

In Short

• Conduct comprehensive telemedicine-based E&M visits to establish care and evaluate medical conditions.
• Partner closely with Grayce’s Care Navigators and interdisciplinary team.
• Document assessments and clinical findings in accordance with Medicare billing standards.
• Provide clinical leadership and guidance for care continuity.
• Stay current on telemedicine best practices and evolving Medicare care models.

Requirements

• Active NP or PA license in good standing with multi-state licensure required.
• Board certification in Family Medicine, Adult-Gerontology, Internal Medicine, or Geriatrics.
• Experience delivering patient-centered care to Medicare beneficiaries.
• Strong knowledge of Medicare guidelines and chronic condition management.
• Active enrollment as a Medicare provider (preferred).
• Experience using telehealth platforms.
• A collaborative spirit and passion for delivering whole-person care.
• Enthusiasm for addressing social drivers that influence medical outcomes.
