Staff Consultative Pharmacist - Remote

Become a part of our caring community and help us put health first. The Consultative Pharmacist 2 consults with patients, pharmacies, and providers to provide patient care in the form of optimal medication therapy.

In Short

• Consult with patients, pharmacies, and providers.
• Provide patient care in medication therapy.
• Work remotely from Pennsylvania or Tennessee.
• Schedule: Monday through Friday 2:30 – 11pm EST.
• End-of-life pain and symptom management.
• Document interventions for outcomes measures.
• Collaborate with nurses on medication care plans.
• Possess excellent communication and interpersonal skills.
• Travel occasionally for training or meetings.
• Compensation includes a bonus incentive plan.

Requirements

• PharmD or B.S. in Pharmacy required.
• Experience with hospice care preferred.
• Strong commitment to patient care.
• Proficient in Microsoft Office.
• Excellent communication skills.
• Superior clinical assessment skills.
• Meet minimum requirements for Humana’s WAH internet policy.
