Overview

The Transplant Clinical Coordinator (TCC) plays a vital role in guiding Chronic Kidney Disease (CKD), End-Stage Kidney Disease (ESKD), and waitlisted patients through the transplant process.

  • Act as a key advocate for patients throughout the transplant process.
  • Facilitate access to transplantation by collaborating with healthcare teams.
  • Support transplant evaluations and optimize wait listing processes.
  • Promote living donor awareness and address barriers to transplant.
  • Enhance patient education and streamline care coordination.
  • Work closely with nephrologists, dialysis providers, and transplant teams.
  • Utilize electronic health records for patient management.
  • Drive better transplant outcomes for CKD and ESKD patients.
  • Collaborate with community organizations to improve access.
  • Ensure seamless support from referral to transplant completion.

Requirements

  • Medical Assistant (MA) or Care Coordination Certificate required.
  • Bachelor’s degree in Health Care Administration or Biology preferred.
  • Minimum of 3+ years of experience in transplant coordination or related fields.
  • Experience with CKD, ESKD, and transplant patients.
  • Knowledge of transplant evaluations and living donor programs.
  • Strong patient advocacy and care coordination skills.
  • Excellent communication and collaboration abilities.
  • Strong problem-solving and critical thinking skills.
  • Familiarity with healthcare systems and community resources.
  • Ability to work cross-functionally with all teams.
