Overview

The Enrollment Specialist II is responsible for outreaching potential members for Health Home Care Management Services, focusing on engaging high-risk members and guiding them through the enrollment process.

Requirements

• Basic computer skills (Windows 365, Outlook, Word, Excel).

• Compliance with quality assurance and safety policies.
• Bachelor's degree in relevant field or equivalent experience.
• Experience with behavioral health diagnoses preferred.
• Valid NYS Driver’s License required.
• Criminal history check required.
