Overview

Chronic Care Staffing is seeking a Remote Care Coordinator (CMA) to join our growing healthcare team. In this role, you’ll help patients manage their chronic conditions through consistent communication and coordination with their healthcare providers.

In Short

  • Conduct monthly Chronic Care Management calls.
  • Perform Health Risk Assessments and Transitional Care Management.
  • Communicate with patients about chronic conditions and treatment plans.

Requirements

  • Certified Medical Assistant (CMA) certification.
  • Experience in chronic care management or healthcare coordination.
  • Strong communication and interpersonal skills.
  • Ability to work independently in a remote environment.
  • Familiarity with EHR systems.
  • Knowledge of CMS guidelines.
