Clinical Documentation Specialist - Remote

Overview

Clarity Partners is seeking a fully remote Clinical Documentation Specialist to improve the quality and completeness of clinical documentation in the legal medical record to support accurate coding, reimbursement, and patient acuity reporting.

In Short

• Facilitate accurate and complete documentation in the medical record.
• Collaborate with physicians, HIM, and coding staff.
• Educate physicians on clinical documentation requirements.
• Apply clinical knowledge to evaluate medical records.
• Perform concurrent reviews of inpatient medical records.
• Initiate queries to obtain missing or unclear information.
• Support appropriate reimbursement and Severity of Illness.
• Ensure compliance with coding and reimbursement updates.

Requirements

• License to practice as a Registered Nurse preferred.
• Credentialed as RHIA, RHIT, or CCS.
• 1-year Acute Care Concurrent Clinical Documentation Specialist experience.
• CCDS or CDIP credential required.
