This temporary position involves managing care for Medicare Dual Eligible Special Needs Plan members, aiming for optimal outcomes through individualized care plans.

In Short

  • Temporary position from September 2025 to December 2025.

  • Responsible for developing comprehensive care plans.
  • Educate members, families, and providers on the D-SNP program.
  • Participate in quality improvement studies.
  • Provide oversight and support for the D-SNP program.
  • Liaison between the program and community agencies.
  • Requires a current RN license in California.
  • Minimum of seven years of patient care experience.
  • Strong critical thinking and problem-solving skills are necessary.

Requirements

  • Knowledge of clinical nursing principles and practices.
  • Experience with case management and motivational interviewing.
  • Understanding of complex chronic conditions and geriatric care.
  • Ability to assess member risk and develop care plans.
  • Experience with electronic health record systems.
  • Effective communication skills with diverse populations.
  • Current unrestricted RN license in California.
  • Associate’s degree in Nursing required.
  • Experience in acute or outpatient settings preferred.
  • Ability to manage complex priorities and document accurately.
