The Hospice Triage Nurse is responsible for answering triage calls during assigned shifts, providing guidance on managing hospice-related symptoms, and collaborating with an interdisciplinary team to ensure seamless care coordination.

Requirements

• Licensed Registered Nurse in the state of practice.
• Hospice or Homehealth experience REQUIRED.
• BLS Certified.
• Excellent communication and interpersonal skills.
• Effective oral communication skills.
• Familiarity with medical records and software.
