Overview

The Clinical Liaison is responsible for providing home health and hospice care coordination and discharge planning for patients, ensuring smooth transitions from hospital to home health and/or hospice care.

In Short

  • Full-time position with a schedule of Monday through Friday, 8am-4:30pm.
  • Coordinates care for patients referred by hospital case managers.
  • Collaborates with healthcare teams to utilize resources effectively.
  • Ensures compliance with federal and state regulations.
  • Communicates with patients and healthcare teams for smooth transitions.
  • Identifies and resolves discharge barriers.
  • Maintains documentation in accordance with department policies.
  • Builds relationships with physicians and referral services.
  • Participates in health education for staff and patients.
  • May include a signing bonus based on eligibility.

Requirements

  • Associate degree required; Bachelor's preferred.
  • Minimum five years of experience as a Clinical Liaison or similar role.
  • Strong verbal and written communication skills.
  • Ability to build effective relationships with referral resources.
  • Experience in hospice and certified home health settings.
