Overview

The Case Management Coordinator is responsible for improving care coordination and enhancing the quality of life for clients through effective care management interventions. This role involves collaboration with clients, families, and healthcare professionals to ensure safety and well-being.

In Short

  • Full-time position, 40 hours/week, Monday-Friday.
  • Fully remote work from home.
  • Conduct assessments and develop action plans for clients.
  • Monitor service implementation and evaluate outcomes.
  • Provide telephonic support for members with chronic conditions.
  • Engage members through coaching and education.
  • Ensure compliance with medical necessity guidelines.
  • Participate in data collection for clinical information.
  • Maintain knowledge of contracts and service providers.
  • Advocate for members and facilitate communication with healthcare providers.

Requirements

  • Associates degree in a related field or equivalent experience.
  • Four years of recent clinical experience in a specialty area.
  • Knowledge of word processing software and quality improvement processes.
  • Strong communication and organizational skills.
  • Ability to work independently and make sound decisions.
  • Active RN or social work license required.
  • Case management certification preferred.
  • Experience in case management strongly preferred.
  • Prior work from home experience is a plus.
  • Analytical and critical thinking skills required.
