Overview

The Case Management Coordinator plays a crucial role in facilitating healthcare outcomes for members by utilizing critical thinking and judgment to support the case management process.

In Short

  • Utilize care management tools to evaluate members’ needs.
  • Coordinate and implement care plans for members.
  • Consult with case managers and medical professionals to enhance quality of care.
  • Provide coaching and support to empower members in their healthcare decisions.
  • Monitor care plan progress and document outcomes.
  • Identify high-risk factors impacting member outcomes.
  • Work independently and collaboratively in a remote setting.
  • Proficiency in Microsoft Office and other corporate applications.
  • Must reside in or near Lake County, Illinois.
  • Full-time position with hours Monday to Friday, 8 AM to 5 PM CST.

Requirements

  • Bachelor's degree in behavioral health or human services required.
  • 2 years of experience in behavioral health or social services preferred.
  • Experience in case management and discharge planning preferred.
  • Excellent analytical and problem-solving skills.
  • Effective communication and organizational skills.
  • Ability to work independently and navigate multiple systems.
  • Confidence in remote work settings.
